Description Hazel Findlay is seeking an MDS Coordinator to coordinate and oversee functions of the MDS and resident care assessment process. We're looking for a compassionate, creative, and resourceful professional to drive our mission. Full-time position available.
Some responsibilities include:
Interview residents and others for information related to the MDS. Review medical records to obtain information related to the MDS.
Review quality indicator reports for resident concerns.
Schedule MDS assessments according to state and federal guidelines.
Schedule and participate in Care Conferences.
RequirementsCurrent LPN or RN license in good-standing with the State of Michigan. Current CPR certification. RAC certification or ability to obtain within 6 months Excellent customer service and interpersonal skills. Some experience in the care and management of geriatric residents.
