JOB SUMMARY
The Medication Reconciliation Technician (MRT) will be independently obtaining medication and allergy histories for patients in the Medical Center Inpatient unit. Information will be obtained by interviewing the patient, family, calling local pharmacies, Long Term Care facilities and other resources. The information obtained will be entered into a computer system and used by physicians, pharmacists, nurses, and other health care providers to determine treatment plans and medication orders during and post discharge from the hospital. The outcome of this interview is a formulation of a complete and accurate medication list that is ready and available for physicians and nurses in a timely manner. The MRT will promote patient safety through medication reconciliation by reviewing all of the home medications so an accurate record is available during transitions of care to avoid unnecessary discrepancies of medications. The MRT will also help facilitate coverage of and access to discharge medications at the discretion of the transitional care pharmacist. The Medication Reconciliation Tech reports to the Transitional Care Pharmacist or designee for program oversight, continuing education & policies.
MINIMUM QUALIFICATIONS
May have State Pharmacy Technician Certification through Pharmacy Technician Certification Board (PTCB). Prefer to have one year pharmacy technician experience in retail or hospital setting.
Professional and Technical Skills:
Experience with Medication classification, math, computer skills and resources.
Education
High School Diploma is a minimum. Pharmacy Technician certificate preferred.
License(s) or Certification(s)
Pharmacy Technician certificate through state or eligible to sit for the certification.
Other Skills or Requirements
Basic Life Support (BLS)
Skilled to receive appropriate delegation of Care for Certain Age-Related Patient Groups (incumbents will be skilled in the care of the following patient groups)
