About

Hospice Nurse - Home Health This is a 13-week contract position. Full-time schedule: 5 days a week, 8 hours per shift, with mainly daytime hours. May include after hours and/or weekends based on agreement. The Hospice Nurse will provide compassionate care to patients in their homes, ensuring comfort and support during their end-of-life journey. This role requires recent experience in hospice care and the ability to navigate HomeCare HomeBase systems effectively. Responsibilities:
Conduct patient assessments and develop care plans. Administer medications and manage symptoms effectively. Provide education and support to patients and families regarding end-of-life care. Collaborate with healthcare team members to ensure high-quality care. Document care provided in HomeCare HomeBase or as trained.
Required Experience / Certifications / Licensure:
Recent experience in hospice care is required. HomeCare HomeBase experience preferred; training may be provided for the first two weeks. Must have active RN license and appropriate certifications for the role.
