Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of care Instructs the clients' family in the care of the client and maintenance of a healthy environment for the client Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the client's condition daily Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Current and unrestricted LPN licensure in state of practice Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation Ability to work flexible hours as required to meet identified patient needs
Preferred Qualifications:
Current CPR certification or ability to complete within 90 days of hire Previous home care experience Ability to work flexible hours as required to meet identified patient needs Able to work independently Good communication, writing, and organizational skills
$51,738 - $77,606 annual total cash target pay $24.87 -$37.31 hourly rate $29.85 - $44.77 per visit point
