Schedule Day shift, 5 days a week from 07:30 AM to 04:00 PM, with weekends required every other weekend. Job Description We are seeking a skilled Registered Nurse or Medical Social Worker with acute care experience to join our Case Management team. The primary focus will be on discharge planning and care coordination within a Level II Trauma Center. The candidate must have experience working across various units, including the emergency department and medical-surgical settings. ResponsibilitiesCoordinate discharge planning and ensure smooth transitions for patients. Conduct thorough assessments and evaluations related to patient care needs. Collaborate with interdisciplinary teams to develop and implement patient care plans. Assist patients in accessing community resources and support services post-discharge. Maintain accurate and timely documentation in the EPIC charting system. Required Experience / Certifications / LicensureMinimum of 1 year of acute care experience in a hospital setting. Experience in discharge planning and care coordination. Current RN Licensure or MSW in the state, with BLS certification required.
