About
Coordinate discharge planning and ensure smooth transitions for patients. Conduct thorough assessments and evaluations related to patient care needs. Collaborate with interdisciplinary teams to develop and implement patient care plans. Assist patients in accessing community resources and support services post-discharge. Maintain accurate and timely documentation in the EPIC charting system.
Minimum of 1 year of acute care experience in a hospital setting. Experience in discharge planning and care coordination. Current RN Licensure or MSW in the state, with BLS certification required. Strong preference for candidates with diverse case management experience.
