About
Support client nursing assessments, including vital signs, medical history, and medication reconciliation. Collaborate with the multidisciplinary team to develop individualized care plans for clients. Administer medications, including injectable medications, and treatments as prescribed by healthcare providers. Performs various health screenings according to established guidelines.
Takes vital signs and performs other tests (such as glucose monitoring, pregnancy tests, UDS, etc.) as required by the prescriber following established guidelines. Collects specimens for diagnostic testing according to established procedures.
Monitor clients for changes in condition and report any concerns to the healthcare team. Provide education and support to clients and their families regarding treatment plans, medications, and self-care techniques. Assist with crisis intervention and de-escalation techniques as needed. Maintain accurate and up to date documentation of client care activities in the electronic medical record. Coordinate with community resources and other healthcare providers to ensure continuity of care for patients. Participate in multi-disciplinary staff meetings, training, and quality improvement initiatives. Adhere to all regulatory and organizational policies and procedures. Other duties assigned by supervising RN, Program Manager, or other members of leadership.
Qualifications Minimum Qualifications: Vocational/Technical degree from an accredited college. Current Georgia license to practice as a Licensed Practical Nurse. Active licensure to practice as a Licensed Practical Nurse in the state of Georgia.
