In this role, you will deliver evidence-based, patient-centered care to older adults, focusing on improving safety, independence, and quality of life. Responsibilities include evaluating functional and cognitive needs, developing individualized treatment plans, providing ADL and mobility training, documenting in EMR, and collaborating with an interdisciplinary team.
