Contract Length: 14 weeks Guaranteed Hours: 40 per week
Requirements
Current State License as a Physical Therapist Proof of current CPR certification Current driver's license; must have reliable transportation and insurance 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting Previous home care experience - Preferred Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program
Roles & Responsibilities
Perform clinical assessments to identify patient needs and appropriateness for therapy. Document and review patient care plans, progress notes, and other clinical records on an ongoing basis. Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on evaluations and medical referrals. Instruct patients and families on transfer techniques and the use of therapeutic equipment safely. Provide education regarding therapeutic procedures tailored to patient age, condition, culture, and learning readiness. Collaborate with an interdisciplinary team and consult with physicians as necessary.
