We are seeking motivated and skilled Outpatient Physical Therapists to deliver outpatient-level care directly in patients’ homes. This unique position offers the opportunity to collaborate closely with orthopedic surgeons and the hospital’s management company, ensuring a streamlined, high-quality care experience from surgery through recovery.
Duties:
* Provide evidence-based physical therapy services to post-surgical and orthopedic patients in their homes
* Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
* Educate patients and their families about treatment options, exercises, and preventive measures.
* Conduct comprehensive evaluations and implement tailored treatment plans
* Document all care in alignment with outpatient standards and regulatory requirements
* Participate in ongoing case reviews and care team meetings
* Work with and supervise PTAs
Experience & Qualifications:
* Current and valid Physical Therapist license in LA
* Minimum 1 year of outpatient orthopedic or home health experience preferred but not required
* If new grad, will provide training and mentorship
* Strong interpersonal and communication skills
* Self-directed, with the ability to work independently in the field
* Reliable transportation and willingness to travel within service area
* Excellent communication skills for interacting with patients, families, and interdisciplinary teams.
