Requirements: - Salary range: $39.00 - $44.00 - 40 hours per week - 10% salary placement fee
Roles & Responsibilities: - Provide direct care to patients, including assessments and interventions. - Collaborate with healthcare team members to develop and implement individualized care plans. - Monitor patient progress and report changes to the nursing team. - Educate patients and families on health management and wellness strategies. - Maintain accurate patient records and documentation. - Adhere to facility policies and procedures.
