Schedule Shift: 3x8-hour days, Wednesday to Friday, 08:30 - 17:00
Assignment Details Typical visits per day: 5-6 Guaranteed hours: Facility may call off up to one shift per pay period (biweekly)
Requirements - Minimum 1 year skilled home health experience - Required certifications: BCLS (BLS) - OASIS experience required - Preferred: Experience with EPIC charting system - Pending license accepted - References: 2 references covering 1 year within last 3 years
Roles & Responsibilities - Provide speech-language evaluations and develop individualized treatment plans - Conduct reassessments and modify plans as necessary - Implement therapeutic interventions for dysphagia and dysarthria - Manage care for patients with CVA and degenerative diseases - Document patient progress and maintain accurate records
