Full-Time Hours: 40 Hours/Week (Flexible Scheduling); Minimum 25 Visits/Week (7 Points/Day) Pay Structure: $95,000 - $115,000 per Year (Based on Pay per Visit Rates and Experience) Sign on Bonus: $5,000 sign on bonus Additional Earnings: $2,000 Quarterly Bonus for Maintaining 90% Average Productivity Coverage Location: Jasper, Indiana & Surrounding Areas (Including Washington, IN) Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
Responsibilities:
As a PT, you'll play a vital role in helping patients regain mobility and independence Delivering physical therapy services per physician's care plan and state regulations Assessing patient mobility, strength, and rehabilitation potential Designing and implementing personalized therapy programs Educating patients and caregivers on therapeutic techniques and equipment Collaborating with interdisciplinary teams to ensure holistic care Supervising PTAs and home health aides as needed Documenting progress and participating in discharge planning
Qualifications What We're Looking For Required:
Active Indiana PT license Degree in Physical Therapy (Bachelor's, Master's, or Doctorate) CPR certification Valid driver's license, reliable transportation, and auto insurance Recent TB test
Preferred:
2+ years of PT experience Home health or community-based care experience Strong communication and organizational skills Ability to build rapport and lead with empathy
