Identifies the patient using the two-unique identifiers described in the hospital's policy before services are provided. Records phlebotomy requisitions on log. Displays courteous, professional manner towards patients, co-workers, nursing and medical staff.

Ability to pass a Level 2 Background Investigation. High School Diploma or equivalency required. Graduate of an accredited Phlebotomy Certificate program required. Current Certified Phlebotomy Technician (CPT) certification through National Healthcare Association (NHA) or ability to obtain within 3 months of hire. At least six (6) months of active phlebotomy and specimen collection experience is required. Basic Life Support (BLS) Certification Ability to deal with diverse and stress related situations in a calm manner. Must be detail oriented with a high degree of accuracy. Must have good organizational and computer technical skills and have the ability to set priorities.
