Provide strategic clinical leadership across all service lines, including ECM, IHSS-aligned services, and Community Supports. Develop and oversee clinical protocols, care pathways, and quality improvement initiatives. Monitor care delivery and ensure alignment with DHCS, Medi-Cal, and managed care plan standards. Lead and scale clinical audits, case reviews, and incident tracking for continuous quality improvement.
Ensure full clinical and operational compliance with CalAIM, Enhanced Care Manager (ECM) & Community Support (CS) requirements. Collaborate with compliance and operations teams to meet documentation, reporting, and audit-readiness standards. Stay current on state and county-level regulatory changes and translate them into internal policy and process updates.
Assist in leading a growing team of Care Managers, Caregivers, Community Health Workers, and Care Supervisors. Provide mentorship and clinical escalation support. Develop and deliver training on clinical best practices, cultural humility, and trauma-informed care.
Define clinical outcome metrics aligned with contract goals and funding requirements. Work closely with the IT/data team to design reports that measure clinical performance, member risk, and intervention effectiveness. Identify trends and implement corrective action plans as needed.
Serve as a key member of the executive team, advising on clinical strategy and representing the voice of care teams. Collaborate with DOO, CFO, and Partnerships teams to support service design, pricing models, and care innovations. Represent Omatochi in clinical meetings with health plans, regulators, and external stakeholders.
Qualifications
Licensed clinician required – LVN, RN, NP, LCSW, LMFT, or equivalent (California license required) 2-4 years of progressive clinical leadership in managed care, care coordination, clinical program management, community health, or home-based services Deep knowledge of Medi-Cal, CalAIM, ECM, and Community Supports Experience managing cross-disciplinary care teams (care managers, social workers, nurses) Track record of leading quality improvement, audits, and regulatory compliance Comfortable working in a fast-paced, mission-driven, entrepreneurial environment Strong communicator and culture-builder with a passion for equity and innovation in senior care
Preferred Skills
Experience with clinical documentation systems (EMR/EHR) Bilingual or multilingual is a plus (especially Spanish, Tagalog, Cantonese, or Vietnamese) Experience in tech-enabled care or startup environments Familiarity with SDOH screening, risk stratification, and whole-person care models
