Execute physician prescribed plans of care Complete compliant documentation of the care provided in the system of record Conduct patient assessments in coordination of care plan Perform various activities associated with daily living Monitor for changes in the patient's condition such as weight loss/gain, self-care abilities, and indicators of disease progression Communicate with family members, physicians, case managers, and other prescribed individuals regarding the health of the patients
$32 - $40 an hour Licensed Practical- Nurse up to $40/hour, based on years of experience. Offers are contingent upon passing a background check.
