Über
Perform comprehensive assessments of patients' needs and coordinate individualized care plans. Provide pain and symptom management consistent with the hospice plan of care. Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy. Collaborate with the interdisciplinary team including physicians, social workers, chaplains, and volunteers. Participate in on-call rotation and maintain accurate documentation in compliance with state, federal, and organizational policies.
Required Experience / Certifications / Licensure:
Current Registered Nurse (RN) license in the state of Utah or compact state RN license. Minimum of 3 years of nursing experience, preferably in hospice, home health, or palliative care. Strong clinical assessment skills and excellent communication, interpersonal, and organizational skills. Current BLS Certification. Valid driver's license and reliable transportation for home visits.
