Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options) Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care Ensures efficiency in the delivery of care to determine immediate priorities and to offer solutions for problems. Makes independent clinical decisions based on knowledge drawn from education and experience Follows unit specific safety regulations and guidelines Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict
Job Requirements Minimum Qualifications: **This position will serve Shasta and Tehama Counties.Travel is required to Tehama County.**
HPNA Certification or hospice RN experience within the last 2 years. (May consider training if no qualified candidates apply and operational conditions exist to support training) Associates in Nursing degree from an accredited school of nursing required Current California RN license required Verbal and written communication skills and computer skills required Current California drivers license/automobile insurance required Current BLS certification required
Preferred Qualifications:
Bachelors of Science in Nursing degree from an accredited school of nursing preferred Completion of ELNEC training preferred Hospice and Palliative Care Nurse certification preferred
