Requirements:
Current Certificate of Clinical Competence (CCC) in Speech-Language Pathology from the American Speech-Language-Hearing Association (ASHA) or CCC eligible. Current state license as applicable. Proof of current CPR certification. Current driver's license. Reliable transportation and insurance.
Roles & Responsibilities:
Conduct evaluations and assess the home environment to identify equipment needs related to speech/language function. Identify functional speech defects and establish a Plan of Care (POC) to enhance the patient's functionality. Administer speech therapy programs utilizing specialized therapeutic techniques and/or equipment. Recommend communication devices/aids as indicated. Collaborate with referring physicians and clinical staff to ensure coordinated care contributes to the total Plan of Care (POC). Document patient care progress and the continuing need for treatment. Educate patients and families/caregivers on tasks and resources that support continued functional independence. Identify needs for additional services and refer as appropriate. May supervise home health aides in procedures related to the speech-language therapy Plan of Care (POC). Participate in case conferences and meetings to ensure coordinated care.
