Title: Nursing Assistant Reports To: RN/State Executive Director Classification: Full-Time FLSA Status: Non-Exempt JOB SUMMARY Responsible for assisting nursing staff to ensure the maintenance of a healthy, safe, and therapeutic environment for individuals served and following through on their individual plans of care. ESSENTIAL JOB FUNCTIONS
Ensure the health and welfare of the individuals served. Assist individuals served with medication and treatment plans as needed.
Assist with doctors' appointments as requested. Communicate all physician notes and recommendations to appropriate staff (ie; RN, Program Supervisor, QIDP, and CHM)to ensure timely follow-through. Deliver medications as needed. Ensure 72 hours of medications are maintained at each location; re-order medications as needed. Ensure expired medications are removed and disposed of according to standards. Document in medical charts and upload required documents in THERAP. Ensure telephone orders are signed within 72 hours of the order being written, and filed within 5 days. Order medical supplies for the homes as needed. Ensure vital signs are documented on a monthly basis. Ensure on a monthly basis that all adaptive equipment for each person is available and in good condition. Organize and maintain medication cabinets, medications, and pill counts monthly (remove discontinued meds, empty blister packs, expired medications, checking physician orders against MARS and labels). Provide training to residents and to staff in the area of medication, including but not limited to self-administration of medication. Maintain proper documentation of residents' records in accordance with Evergreen's policies and procedures. Assist in preparing and maintaining medication administration records and current physician's orders for all individuals. Assist in the preparation of referral forms and other documents as requested. Assist in scheduling appointments with medical professionals as needed. Represent Evergreen in a professional manner and act as Evergreen's liaison with the medical community. Ensure timely filing and follow-through with lab results and other medical reports. CORE COMPETENCIES

QUALIFICATIONS/EXPERIENCE/JOB KNOWLEDGE
High School diploma or equivalent. Prior work in a non-profit atmosphere and experience with people with intellectual and developmental disabilities are preferred. A combination of education and experience will be considered. PHYSICAL REQUIREMENTS
Constantly moves about to coordinate work. Regularly works in a fast-paced environment with multiple task deadlines. Regularly moves and positions objects weighing up to 50 pounds. Occasionally exposed to viruses and infectious conditions. Constantly alert and observant during working hours. Reasonable accommodations may be made to enable individuals with disabilities or medical conditions to perform the essential functions. SUPERVISORY RESPONSIBILITIES
Will not supervise. SPECIAL REQUIREMENTS
Knowledge of basic medical standards (federal and state) in ICF/IID and HCBS Waiver programs. Strong understanding of documentation requirements for service delivery. Ability to maintain, organize, complete, and keep medical records compliant. Must have understanding, patience, and tact in dealing with individuals with intellectual and developmental disabilities. Must have the ability to maintain good working relationships with Evergreen staff and with contacts from other agencies or entities. Must have the ability and desire to function as part of a team, yet work with a high degree of independence. Must have the ability to maintain a high degree of autonomy, accuracy, and integrity. SKILLS AND ABILITIES
Working knowledge of Microsoft Windows and Office applications. Working knowledge of Google Workspace. Working knowledge of ICF/IID and HCBS Waiver requirements. Working knowledge of THERAP. EMPLOYMENT VARIABLES
Must have a good driving record that meets the minimum requirements for Evergreen and reliable transportation. Must pass a drug screen and criminal background check. Availability for evening and weekend work may be required based on operational needs. WORKING ENVIRONMENT
