About
Deliver nursing care under the supervision of an RN in a skilled nursing and rehab setting
Administer medications and treatments per physician orders and care plans
Monitor vital signs, symptoms, and changes in condition
Maintain accurate clinical documentation using electronic medical records (EMR)
Support post-acute care and recovery goals for short-term rehab residents
Participate in care plan meetings and collaborate with interdisciplinary teams
Uphold infection control and resident safety protocols
Qualifications:
Valid LPN
New graduates encouraged to apply – training available
Prior experience in long-term care, rehabilitation, or skilled nursing preferred
Strong clinical, documentation, and communication skills
