An innovative, community-based Adult Day Health Care Program in Manhattan is seeking a compassionate and experienced NYS Licensed Registered Nurse (RN) to join their multidisciplinary team. This program provides vital services to adults with chronic medical conditions and vision impairments-giving you the opportunity to truly make a difference in patients'lives in a supportive, team-oriented environment. What You'll Do: Conduct comprehensive nursing assessments using patient history, interviews, and physician reports Develop, implement, and evaluate individualized nursing care plans Monitor medications and ensure proper administration during program hours Provide health education to participants and families (individual + group sessions) Respond to medical emergencies and guide staff on protocols Maintain inventory of medical supplies and ensure equipment is in optimal condition Collaborate with interdisciplinary team members to coordinate care Act as liaison between program, physicians, and Medical Director Participate in Quality Assurance & Performance Improvement initiatives Assist with daily program operations including participant support, meals, and mobility assistance What We're Looking For:
Active New York State RN License
Graduate of an accredited nursing program
3-5 years of recent RN experience (Med/Surg, Long-Term Care, or Community Health preferred)
Strong clinical assessment and documentation skills
Excellent communication and teamwork abilities
Proficiency in MS Word / computer systems
UAS-NY Certification (preferred)
