Description: The Charge Nurse is responsible for assessing the patient's physical and psychosocial condition on an ongoing basis throughout the continuum of care. Developing a plan of patient care reflecting significant nursing diagnoses and interventions to achieve desired patient outcomes is essential. This role involves implementing the patient plan of care in collaboration with the healthcare team. Evaluating patient outcomes, the effectiveness of the plan of care, and readiness for discharge is part of the responsibilities. Revisions to the plan of care are made as needed, along with referrals when indicated. Accurate and timely documentation is required for all patient care activities. Utilizing manual and automated information systems to improve efficiency and productivity is expected. Demonstrating a current working knowledge of equipment, instrumentation, and medications is crucial. Adhering to the standards, policies, procedures, and goals of the organization is a requirement. Acting as a liaison to coordinate patient care activities with nurses, physicians, and allied health professionals is essential. Communicating the patient's plan of care and significant changes to all members of the interdisciplinary team, as well as to the patient and family, is expected. Participating in and supporting performance improvement and risk management activities is part of this role. Providing patient and family education based on assessed learning needs is required. Demonstrating cost-effectiveness by using supplies judiciously and making recommendations for changes is expected. Completing required continuous training and education is necessary. Complying with established personal protective equipment requirements for safety is crucial. Regular attendance and punctuality are required. Performing other assigned duties as necessary is part of the role.
In emergency room coverage, the nurse initiates assessment based on the patient's presenting problems or needs within five minutes of admission. Key information is collected from patients, families, and laboratory reports. Implementing T-System and documenting assessments, including nursing diagnoses or clinical judgments, is required. Ongoing assessments are completed as determined by patient condition. Therapeutic nursing interventions as ordered by the provider, consistent with standards of care, are performed. The nurse determines the impact of nursing interventions on the patient's condition and recognizes the patient's progress towards meeting desired outcomes. Documentation of care is based on standards of care and practice and clinical priorities. Managing the care of patients with conditions in the emergency room is critical. Interacting with patients and families from various backgrounds is required. Providing information to patients and families reduces anxiety and conveys sensitivity and caring. Maintaining professional relationships and conveying relevant information to other healthcare team members is necessary. Relaying information over communication devices is expected.
This position directly supervises licensed practical nurses, certified nurse aides, and ward clerks.
Required education and experience include a current Registered Nurse License in good standing. BLS certification, along with ACLS, TNCC, and PALS, is required within six months of hire.
Preferred experience includes one year as an RN.
