Intake Coordinator The Intake Coordinator is responsible for providing clients with timely access to our home health and mental health services. This includes scheduling intake appointments, ensuring required documentation is completed accurately and providing follow through to ensure that clients are engaged in services. The Intake Coordinator will communicate with operations to ensure caseloads are updated and services are being provided. Primary Job Responsibilities:
Serve as the primary point of contact for all new clients seeking home health or mental health services. Verify eligibility for Mental Health and Home Health services and inform clients of the ability to receive both services. Assist new clients with enrollment into the waiver programs to become eligible for services Communicate new enrollments with the Director of Operations and Field Supervisors. Collect new client information and create client profiles with pertinent information in EHR. Schedule intake appointments with clinicians for referrals received by phone or online. Collect intake consents/authorization renewals before referral's intake appointment Collect information related to PCP, emergency contact, previous hospitalizations, etc. to begin process of continuity of care. Set Health Watchers for intake and authorization renewals Reschedule intakes that no-show and document the follow up Update clinician caseloads to reflect new clients Update intakes as inactive in EHR that are not able/do not wish to be rescheduled Track intake show rates, discharges and client retention weekly Collaborate with clinical supervisors, front desk, billing, QA departments, and other providers as needed to ensure accurate documentation and continuity of care.

Strong written and verbal communication skills Teamwork orientation Able to take and follow through with delegated tasks and accountability Proven data analysis skills, organizational skills, and resourcefulness in problem solving Intermediate proficiency in Microsoft applications: Excel, Word, Outlook, PowerPoint Strong attention to detail and follow up Show a high level of care, compassion, understanding and urgency in order to meet all of the client's needs Must be able to maintain client privacy per HIPAA laws
