Care Transition Volunteer Program

About

The Care Transition program has been established to ease the burden of our community members who lack adequate family or caregiver resources and transportation assistance when transitioning from long-term care facilities back to their home and through recovery. Volunteers will be matched with a client for a three-month or 60 hour period to offer:
Companionship Respite In-home support, such as light homemaker services and fall prevention Transportation to non-emergency medical or pharmacy appointments Assist staff with in-home client assessments
Transportation home from the long-term care facitlity
Volunteers have a right to say "no" to any services that is requested. Staff will work with clients when volunteers wish to not assist with certain services. Volunteer in the program receive:
A stipend of up to $500 reimbursed at a hourly rate for services completed Passenger Mileage reimbursement at $0.70 per mile Meal reimbursement for both themselves and the client
