About
Perform therapeutic procedures under the direction of a licensed therapist, following physician orders and established care plans. Implement and support individualized treatment plans, including functional goals, frequency, duration, and therapeutic modalities. Monitor patient progress, report status changes, and collaborate with the supervising therapist to modify care plans as needed. Provide hands-on care while ensuring adherence to all safety precautions, indications, and contraindications. Engage residents and caregivers in the care plan, providing education and documenting outcomes effectively. Maintain accurate and timely documentation in compliance with department and regulatory standards. Participate in team meetings and contribute to coordinated patient care. Ensure compliance with Medicare, Medicaid, and other reimbursement guidelines. Assist in maintaining a clean, safe, and infection-controlled clinical environment. Support continuous improvement initiatives and professional development activities. Requirements: Associate Degree (required) Active Physical Therapy Assistant (PTA) License Basic Life Support (BLS) Certification (required) Prior experience in long-term care or skilled nursing facility (preferred) Strong communication, interpersonal, and teamwork skills Ability to manage time effectively and work in a fast-paced environment Proficiency in Microsoft Office Physical Demands: Ability to perform tasks involving standing, walking, bending, and lifting Must be able to lift up to 50 lbs (assistance required for higher weights)
