In der Spitex:
Ganzheitliche Pflege und Betreuung unserer Klient:innen zu Hause
Erstellen von Pflegeplanungen und Durchführung von Pflegevisiten
Fachliche Beratung und Schulung von Pflegefachkräften und Angehörigen
Sicherstellung der Pflegequalität durch evidenzbasierte Praxis und kontinuierliche Verbesserung
In der Hausarztpraxis:
Unterstützung des Hausarztes bei der Versorgung und Betreuung der Patient:innen
Durchführung von Gesundheitschecks und präventiven Massnahmen
Mitwirkung bei der Diagnose und Therapie von chronischen und akuten Erkrankungen
Patientenedukation und -beratung, insbesondere im Bereich Gesundheitsförderung und Krankheitsprävention

Das bringst du mit
Abgeschlossene Ausbildung zur/zum Advanced Practice Nurse (APN) oder gleichwertige Qualifikation
Mehrjährige Berufserfahrung in der Pflege, idealerweise in der Spitex und/oder in einer Hausarztpraxis
Hohe Fach- und Sozialkompetenz sowie die Fähigkeit, eigenverantwortlich und selbstständig zu arbeiten
Ausgeprägte Kommunikationsfähigkeiten und Freude an der interdisziplinären Zusammenarbeit
Flexibilität und Bereitschaft, sich auf die Bedürfnisse unserer Klient:innen und Patient:innen einzustellen
