And everyone, in their own unique way, plays an important part, every day./ppThis position provides routine, personal care such as bathing, dressing, or grooming, to convalescent, terminally ill, or disabled persons in their home setting./ppI. Major Responsibilities:/pulliProvides direct and indirect personal care in a safe and timely manner in accordance with departmental policies and procedures./liliPerforms homemaking and other environmental services in a way that facilitates sanitary and safe care in the patients home according to policies and procedures./liliDuties may include:/liulliProviding patients with mobility assistance, bathing, dressing, and grooming./liliProviding patients and families with emotional support and instruction in areas such as caring for patient or self, preparing healthy meals, living independently, or adapting to disability or illness./liliChanging bed linens, washing patients laundry, cleaning patients living area and other homemaking tasks identified within the home health plan of care./liliPreparing or serving meals to patients according to the home health plan of care./liliDirecting patients in simple prescribed exercises or in the use of braces or artificial limbs./liliChecking patients pulse, temperature, and respiration./liliChanging clean dressings./liliPerforming a variety of duties as outlined in the home health plan of care./li/ulliMaintains records of patient care, condition, progress, or problems to report and discusses observations with supervisor or professional according to policy./liliObserves, documents, and reports changes in client status or expectations of services promptly to a supervisor or professional according to policy./liliRecognizes emergency situations and implements interventions according to appropriate departmental procedures and policies./liliRenders services in accordance with the written plan of care as documented by the appropriate professional./liliStandard Staffing Level Responsibilities:/liulliComplies with established departmental policies, procedures and objectives./liliAttends variety of meetings, conferences, seminars as required or directed./liliDemonstrates use of Quality Improvement in daily operations./liliComplies with all health and safety regulations and requirements./liliRespects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors./liliMaintains, regular, reliable, and predictable attendance./liliPerforms other similar and related duties as required or directed./li/ulliAll responsibilities are essential job functions./li/ulpII. Position Qualifications:/ppLicense/Certification/Education:/ppRequired:/pulliCurrent Home Health Aide certification or Certified Nursing Assistant (CNA) license./liliMust have a valid US-issued drivers license and a registered, inspected, and insured automobile for work related purposes./li/ulpPreferred:/pulliHigh School Diploma or equivalent./li/ulpExperience/Skills:/pulliRequired:/liulliMinimum six (6) months of experience providing care as a certified home health aide in a home-based setting./liliExcellent oral and written communication skills./liliThe ability to read, write and speak clearly in English./liliAbility to listen and give full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times./liliAbility to develop and maintain constructive and cooperative working relationships with patients, coworkers, and supervisors./liliAbility to consistently be responsible, compassionate, patient, emotionally stable, and cheerful with patients, caregivers, and other professionals./liliAbility to be tactful, honest, and respectful of patients and family members in private homes./li/ulliUnless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements./liliDepartment-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents./li/ulpIII.
