A community hospital in Nyack, NY is looking for a Manager of Case Management to join their team!
Job Description: Compensation : $ 140000

Job Function:
Leads and oversees the hospital's care management function, ensuring patients receive effective, high-quality care while maintaining efficiency and cost awareness.
Provides leadership and supervision to case management staff, ensuring patients are supported throughout their entire care journey - from admission through discharge and beyond - while promoting continuity of care and positive outcomes.
Works closely with physicians, care teams, patients and families, and payers to coordinate services and support optimal outcomes.
Guides decisions related to patient length of stay by applying knowledge of regulatory requirements and payer guidelines, helping to facilitate timely and appropriate discharges.
Job Requirements: Job Requirements:
BSN required
Current NYS RN license
5+ years of RN experience in acute care setting, including 2+ years experience in case management or utilization review
1+ years of leadership experience
Case Management and/or PRI certification preferred
