Job Description
The Outpatient Mental Health Therapist delivers exceptional, patient-focused care by providing clinical services in a multidisciplinary team.
Duties and Responsibilities- Conduct one-on-one and group psychoeducational and process groups focused on skills/resilience training and evidence-based treatment.
- Develop treatment plans and attend treatment planning meetings and reviews on an ongoing basis.
- Coordinate with existing outpatient therapy care teams.
- Assess environment and patient's safety status and report/respond to actual and potential safety issues or changes in the patient's condition.
- Identify risk factors, screening criteria, and interventions for patients with special precautions such as suicide, homicide, falls, and elopement.
- Complete assessments, group notes, and all other documentation promptly and in compliance with requirements.
- Collaborate with the multidisciplinary team to assess and coordinate patients' discharge planning needs and readiness for discharge.
- Conduct post-discharge follow-up as indicated.
- Basic Cardiac Life Support Certification
- Nonviolent Crisis Intervention Training
- Masters Degree in Psychology, Counseling, or Social Work is Required
- At Least 3 Years of Therapeutic Experience is Strongly Preferred
- Psychotherapy Experience is Required
- Advanced Licensure in the State of California (LMFT, LCSW, LPC, or Psychologist) is Required
