Conduct comprehensive assessments of residents' physical and mental health upon admission and regularly thereafter. Develop, implement, and monitor individualized care plans in collaboration with residents, families, and healthcare providers. Administer medications, treatments, and wound care according to established protocols and physician orders. Monitor residents' conditions, vital signs, and medication administration, documenting findings accurately and timely. Collaborate with interdisciplinary team members, including LPNs, CNAs, and therapists, to coordinate resident care. Provide education and support to residents, families, and staff regarding health conditions, medication management, and self-care. Participate in quality improvement initiatives and adhere to infection control practices. Ensure compliance with state and federal regulations, as well as facility policies and procedures. Build strong relationships with residents and their families, providing emotional support and addressing their concerns.
Qualifications:
Current and valid RN licensure in the state Bachelor of Science in Nursing (BSN) preferred. Basic Life Support (BLS) Minimum of 1 years of experience in assisted living or skilled nursing facility preferred. Strong clinical assessment and problem-solving skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Compassionate and patient-focused attitude.
