Provide care in nursing and long-term care facilities Manage chronic conditions and perform full assessments Close care gaps (HEDIS and STAR measures) Lead care planning with facility teams Support and educate patients' families Must-Haves
Active, unrestricted APN license in Michigan Board certified and Medicare credentialed 6+ months in skilled nursing or long-term care Solid with digital tools and team-based care Able to work onsite in Saginaw, MI Perks & Pay
