About
Deliver skilled nursing care and support the development and execution of individualized, goal-oriented care plans. Perform ongoing patient assessments and promptly report changes in condition to the supervising RN or Clinical Manager. Administer medications and treatments accurately, following established protocols and maintaining strict aseptic technique. Provide patient and family education on treatments, health management, and self-care techniques. Document care clearly and on time in accordance with agency and regulatory standards. Participate in interdisciplinary team meetings and quality improvement activities. Promote patient rights and adhere to HIPAA, infection control, and safety protocols. Collaborate effectively with the healthcare team, orient new staff as needed, and model professional conduct. Manage time effectively to meet clinical and administrative requirements while maintaining high standards of care.
Qualifications
Current, active LPN license in the state of practice, in good standing. Minimum of one year of clinical nursing experience preferred; home health experience is desirable. Strong communication and documentation skills. Ability to work effectively as part of a healthcare team. Commitment to safe, compassionate, and patient-centered care.
Work Environment The role involves patient care in home, assisted living, or clinical settings. Physical activity includes standing, walking, bending, reaching, and occasional lifting or transferring of patients with proper body mechanics and assistive devices. Candidates must be able to lift, carry, push, or pull up to 35 pounds and respond quickly in emergencies. The position involves routine exposure to potentially infectious materials; adherence to infection control procedures and use of PPE is required.
