Employment Type: Full time Shift: Day Shift Description: Full time RN Case Manager in the inpatient setting at Trinity Health Grand Rapids Hospital This position is for a RN Case Manager at Trinity Health Hospital in Grand Rapids, MI. Trinity Health is a Magnet Designated Hospital! This role will provide case management to our patients and families in the acute care setting. Oversees and coordinates the discharge planning and utilization management activities for assigned caseload. Facilitates appropriate utilization of resources and efficient patient progression through the continuum of health care resulting in the highest quality and most cost-effective care. What you will need:
Minimum: BSN; or ADN, with BSN to be obtained within 4 years of hire
Current RN license in the State of Michigan.
Preferred: Certification in Case Management or other related professional certification.
At least 2 years of experience in Case Management experience in an acute care hospital
Preferred: 5+ years of Case Management experience
