Review and administer meds according to physician orders. Ensure accuracy of medication administration records (MAR)
-Update the MAR as needed
Ensure correct prescriptions have been uploaded.
-Ensure prescriptions are updated at least annually
Ensure proper storage and destruction of all medications
-Order medications 7 days prior to running out -Pick up medications when needed -Monitor seizure activity -Monitor for changes in weight (If Congestive Heart Failure be sure to include heart and lung sounds) -Monitor nutrition (Intake) -Monitor bowel and urinary status (Output): Enter note in Bowel tracker every 7 days. Notes should reflect whether bowel movements were within 3-day protocol and if additional PRN medications were needed.
Ensure bowel interventions/management as prescribed. Monitor and document skin integrity daily (Check for injuries) Perform preventative measures for skin break down
-Monitor fall risks -Check for any issues with ambulation -Check for illnesses -Check blood pressure -Check pulse -Check respirations -Perform head to toe assessment -Complete HRST Data Tracking (VRST) -Review appointment information and ensure appropriate follow-up with PCP, labs and other specialties has occurred -Review Appointment Summaries and Discharge Notes from Hospitals -Review and complete Appointment Education Sheet after appointments -Destroy medications as needed and enter information on medication destruction sheet -Review labs -Enter appropriate notes in Vypersoft -Other duties as assigned. Weekly LPN Tasks -Every 7 days sign off on the BM tracker. KNOWLEDGE, SKILLS & ABILITIES: 1-3 years of any combination of training and experience working in a hospital, private duty nursing or other types of mental health facilities 6-months of experience providing social or direct care services to groups of special needs individuals in a human service setting Must have valid driver’s license, current automobile insurance and social security card Good communication and interpersonal skills Ability to follow oral instructions Ability to prioritize multiple tasks Ability to work independently Knowledgeable of computer for e-mail, internet, data entry QUALIFICATIONS: Successful completion of a formal educational program in practical nursing approved by the Georgia Board of Nursing. Successful passing of the National Council Licensure Examination for Practical Nurses and a current Georgia issued license. Successful completion of orientation and competency-based training provided by the provider agency and State of Georgia. WORKING ENVIRONMENT: The work is performed in B & O Services, Inc's. personal care home/s. PHYSICAL DEMANDS: The work requires routine walking, standing, bending, and lifting at least 50 pounds.
